Mary Greeley Medical Center Grand Rounds is a multi-disciplinary clinical activity which serves to maintain, develop, or increase the knowledge, skills, and professional performance and relationships that a physician uses to provide services for patients, the public or the profession.  

Target Audience

Physicians, mid-level providers, nurses and healthcare administrators.

Learning Objectives

1. Define advanced care planning (ACP) & why current systems don't work.
2. Describe the four key elements & related strategies in designing an effective ACP program for all populations.
3. Provide evidence-based research related to effeciveness of an ACP model.
4. Review outcomes & benefits of a systematic, standardized approach to discussing ACP with the patient & family.
