Target Audience
Healthcare professionals.
Educational Need
Providing culturally aware services to support patients –including cognitive concerns- into primary care other medical providers who are eligible to complete N-648 form. Medical providers are often requested to complete the N-648 form for their patients, but there are often questions and concerns about filling out the form and completing this correctly per federal requirements. Due to many providers not understanding form completion expectations, many do not agree to complete the form, which can create barriers and hurdles for patient who are eligible for the waiver.
The Broadlawns Hospital, Lutheran Services of Iowa, and Primary Health Care Inc. organizations participating in the training - have partnered to support this patient community. This is difficult work and learning from peers in this field will help support other medical providers to feel more confident in completing waivers for their patients.
