IPOST: Putting Impactful Conversations into Action

Purpose

The IPOST form is a dynamic and actionable provider order form used in Iowa for over a decade. It is part of the national POLST movement, pushing to drive critical conversations through shared decision-making and capture them in a provider order that is transportable and actionable toward the patient's spoken goals and wishes. Understanding how to utilize the form is paramount to helping defragment a health system often riddled with communication challenges between providers.

Learning Objective

  1. Review the POLST paradigm movement and Iowa’s IPOST.
  2. Discuss the power of the IPOST conversation.
  3. Use the AsK, Teach, Ask model to have difficult conversations.
  4. Identify common pitfalls in difficult conversations.
  5. Identify common pitfalls with the IPOST form.
  6. Learn strategic language for navigating critical conversations.

Speaker

Tom Mouser, MD, has been a tertiary Hospice and Palliative Care physician as the Chief Medical Officer at EveryStep for the past 12 years.  He is dually boarded in Internal Medicine and Hospice and Palliative Care after completing his residency at the University of Iowa-Des Moines and fellowship training at Stanford University.  Dr. Mouser has spent countless hours at the bedside of those with serious illnesses and chronic diseases, helping navigate complex medical decisions guided by the patient’s goals of care. He has served on the Iowa State IPOST task force, helping promote understanding and better utilization since its inception.
