Purpose
Participants will enhance their skills and knowledge of Medication Management for Hospice patients: including understanding what is a Pharmacy Benefit Manager and how is it used, understanding of and how to determine appropriate deprescribing, and related/unrelated medications, and implementation of new regulations. Additionally, they will learn about the change in relations related to the NOE and information available for patients and caregivers.
Learning Objectives
- Describe what a Pharmacy Benefit Manager (PBM) is and how to use it.
- Define deprescribing.
- List the categories of medications to be considered for deprescribing.
- Discuss the risks and benefits to deprescribing.
- Describe the utilization of each deprescribing algorithm.
- List the new requirements for the Hospice Election Statement.
- Summarize the regulatory changes.
