Purpose
To increase participation among Medicare beneficiaries in Medicare-covered diabetes prevention/management programs.
Learning Objectives
- Discuss the evidence for Diabetes Prevention Program (DPP) and the Diabetes Self-Management Education and Support (DSMES).
- Review the eligibility criteria for DPP and DSMES.
- Describe the referral process for DPP and DSMES.
