The activity director(s) and planning committee request your authorization to record your presentation(s) and publish the recording for viewing outside of the live activity. By authorizing this request, the content can be an educational reference for those who attended and wish to refresh or review the material and/or allow those whose schedule did not permit them to attend live the opportunity to view the presentation and review the educational content. This is optional, and we respect your decision if you decline our request.


Video/Audio Recording Authorization and Distribution Agreement

In consideration of taking part or participating in teaching situations and encounter groups, and in the interests of science and the furtherance of medicine, Speaker consents to and authorizes Des Moines University, its employees, agents, successors, and assigns permission to use and reproduce the presentation and all related materials in any way Des Moines University deems appropriate.

Speaker grants Des Moines University an unlimited, permanent, nonexclusive license, under which Des Moines University may photograph, record (audio and/or video) tape, transcribe, otherwise record or quote from the presentation or materials, may create derivative works, may publish them in any format, including audio recordings, distance education courses, the Internet or any other digital network including any successor technologies, or any other media, and may use them in any other way it may find appropriate without further/prior written consent. The Speaker will be cited as the source in any circumstances noted above.  Des Moines University will not be required to pay royalties, or honoraria, make any other payment, or provide further consideration except as described in this agreement.
  
Speaker warrants and represents that the presentation(s) does not violate any proprietary or personal rights of third parties (including, without limitation, any copyrights, trade rights, privacy rights, or rights of publicity); the presentation is factually accurate; the presentation contains nothing of a libelous, defamatory or unlawful nature; nothing in the presentation promotes or disparages any company, individual, products, services or technologies; the presentation is original to the undersigned; and he or she is the sole owner of any copyright in and to the Work. The Speaker has obtained permission to use, reproduce, and distribute, allowing Des Moines University to use, reproduce, and distribute those materials and information.

The Speaker further warrants and represents that he or she has full authority to grant this License and Release; he or she has not previously granted any rights adverse to or inconsistent with this agreement; and he or she has obtained all permissions from third parties necessary to license the presentation. Speaker permits Des Moines University to use their name, likeness, photograph(s), and biographical information to promote, reproduce, and distribute Des Moines University Continuing Medical Education materials. I further agree to hold Des Moines University, its employees, agents, successors, and assigns free and harmless from any claims, demands, or suits for damages that I or anyone may now have or ever claim to have use of the above materials. I also understand that Des Moines University does not claim my presentation's content (s) validity.
