The Immanuel Pathways PACE (Program of All-Inclusive Care for the Elderly) offers whole-person wellness for seniors qualifying for nursing home care yet still living in their home. PACE is funded by Medicare and Medicaid and provides all the care and services authorized by the PACE team. With PACE, they will receive services exclusively from Immanuel Pathways-a single source for all services. Participants are able to remain living independently for as long as possible and Medicaid qualifying individuals receive services at no cost. Participants and/or families no longer have to call separate providers to ensure everyone's on the same page as we're always working as a team.

Immanuel Pathways PACE®

Immanuel Pathways PACE centers offer enriching activities and friendships along with complete medical needs. Participants are able to remain living independently for as long as possible and Medicaid qualifying individuals receive Pathways services at no cost.  An interdisciplinary team of experts work with seniors and their family to develop the most effective plan of care. Services are provided in their own home, in the community, and at the Immanuel Pathways PACE center.
