As part of a request for accommodation(s), you must submit healthcare documentation. While the following documents are required, submission of only these documents may be insufficient to complete the interactive process. Students are encouraged to submit any information they believe is relevant. The required documents include:
- Completed Student Request for Accommodations form. (must be logged into student account to access). This form initiates the interactive process. An Accommodations Specialist can assist in completing this form if the nature of the impairment would prevent the applicant from successfully completing the request for accommodations.
- Documentation of the presence of an impairment that substantially limits a major life activity of the student from a qualified professional. A clear link between the requested accommodation and how it assists in gaining access to the curriculum must be made
Documentation should address the following:
- A formal diagnosis
- How the impairment impacts major life activities (personally and/or academically)
- Recommended accommodations
- Expected duration (new, lifelong)

If a student cannot provide the information above, the student should email an Accommodations Specialist to determine whether alternative steps can initiate the interactive process.
