Always good to see you John, even virtually! 09:27:53 From Narmo Ortiz to All panelists : I have a 69 yo obese, neuropathic diabetic male referred to me from the wound care center who has an open wound on his posterior heel due to a migrating screw from an IM nail applied during a revision of a failed ORIF of an ankle fracture. Both surgeries were done by another colleague. X-rays show a distally migrating IM nail with completely dislodged proximal nail screw, talar screw, and diagonal abutting of the nail. He's admitted for surgery: plan is to remove all hardware, do bone biopsies on the calcaneus and distal tibia (tibial plafond obliterated), and apply an antibiotic rod spacer intramedullary and apply ex fix. I love the idea that I can leave that antibiotic rod permanently.
